Health Care Services is hiring an RN for our non-skilled 60-day nursing assessments. We are a family-owned Medicaid home care agency serving clients in Providence and the surrounding cities.

Responsibilities:

• Travel to client's home to conduct RI State Mandate 60-day plan of care reviews and CNA supervisory visits. Complete Minimum Data Set-Home Care (MDS-HC) and agency paperwork. Record visits in our computer system.

• Admit new clients to the agency. Use clinical judgment to create a plan of care for the CNA to follow in the home.
• Supervise CNA in the client's home and conduct yearly evaluations.
• Contribute to the agency by facilitating relationships between referral sources, clients, families, and staff.
• Must be willing to travel and work in Providence and surrounding cities and towns.

Skills & Qualifications:

• RN with an active RI license
• Home Care experience is a plus.

• Strong organizational skills and ability to work independently

• Excellent communication skills, both written and verbal.
• Spanish speaking a plus.

Work Schedule:

• Flexible to create your own schedule.

• No nights, weekends, or on-call.
• Must report to the office at least twice a week. Office hours Monday-Friday 8:30-4:30
